INVITATION TO SELF-IDENTIFY TO PERSONS WITH DISABILITIES
The University of Louisiana at Lafayette prohibits discrimination in accordance with applicable federal, state, and local laws, including Title VI and Title VII of the Civil Rights Act of 1964, the Age Discrimination in Employment Act of 1967, the Age Discrimination Act of 1975, the Equal Pay Act of 1963, Title IX of the Education Amendments of 1972, Section 503 and Section 504 of the Rehabilitation Act of 1973 (as amended), Section 402 of the Vietnam Era Veterans' Readjustment Assistance Act of 1974, and the Americans with Disabilities Act of 1990.
The University is subject to specific recordkeeping and reporting obligations under Section 503 and the ADA for civil rights compliance purposes. As a federal contractor, the University is required to periodically survey employees and applicants regarding their disability status using the form developed by the Office of Federal Contract Compliance Programs (OFCCP), titled
“Voluntary Self-Identification of Disability” (Form CC-305, OMB Control Number 1250-0005).
It is the policy of the University not to discriminate against any employee or applicant because they are qualified individuals with a disability. All employment decisions are based solely on valid job requirements.
This policy applies to all aspects of employment, including recruitment, hiring, compensation, promotion, transfer, layoff, return from layoff, and termination.
Applicants and employees are encouraged to voluntarily self-identify their disability status at any time. Applicants may provide this information through the University’s online application portal. Employees may do so through the University’s employee self-service system (accessible via ULink), or by submitting the completed form in person at Louisiana Immersive Technologies Enterprise (LITE), Suite 239, 537 Cajundome, Lafayette, LA 70506.
Participation is voluntary, and refusal to provide this information will not result in any adverse treatment. All responses are confidential and used only in accordance with applicable laws.

Why am I being asked to fill out this form?
The University of Louisiana at Lafayette is a federal contractor and is required to maintain and analyze data on the veteran and disability status of employees, as part of its compliance obligations to the Office of Federal Contract Compliance Programs (OFCCP).
In accordance with OFCCP regulations, federal contractors and subcontractors must use the designated “Voluntary Self-Identification of Disability” form (Form CC-305, OMB Control Number 1250-0005) to collect disability status information at both the pre-offer and post-offer stages of employment.
This data collection helps ensure compliance with Section 503 of the Rehabilitation Act and supports ongoing efforts to provide equal access and opportunity in the workplace for qualified individuals with disabilities.

How will this information be stored?
Your “Voluntary Self-identification of Disability Form” will be stored separately from your personnel file.
Do I have to fill out this form?
No. Completing this form is voluntary, but we hope that you will choose to fill it out. Because the University is considered to be a government contractor, we must document our efforts to provide equal opportunity to qualified people with disabilities. To help us measure our efforts, we are asking you to tell us if you have a disability or if you ever had a disability. Your individual information will be kept confidential.
What if I need to update my disability status?
Because a person may become disabled at any time, you may submit an updated form as needed.
