Background Check Forms

The Office of Human Resources has simplified the University's background check process for both classified and unclassified employees. Effective Monday, May 22, 2017, the background check process will be handled electronically. Paper applications will no longer be accepted.

Hiring managers will email the candidate's first and last name and email address to to initiate the background check. Human Resources will honor applications that are already in process prior to May 22nd.
