All employees assigned or approved to telework must complete the Remote Office Checklist with their Supervisor to ensure they have the resources and tools to telework effectively.

Teleworking Request Worksheet
Supervisors will assess employee requests to telework by completing the Teleworking Request Worksheet.

Teleworking Request Form
If the Supervisor determines a position is suitable for telework, then the Supervisor will complete the Teleworking Request Form and submit to the appropriate Vice President for approval.

Remote Office Checklist
Department heads will complete the Remote Office Checklist to ensure their employees have the resources and tools to telework effectively.
