Blue Cross and Blue Shield of Louisiana has engaged Catapult Health to provide
1095-C forms mailed to employees and available electronically
IRS Form 1095-C: Employer Provided Health Insurance Offer and Coverage has been mailed to all employees who were eligible for health insurance through the university in 2017. This form is not required to file your taxes, but should be kept with your 2017 tax records.
To view the form electronically, access Banner Self Service via ULink and click the Employee tab. Select Tax Forms, then Electronic Regulatory Consent. Choose the option for Form 1095-C, return to Tax Forms, and select 1095 Employer Provided Health Insurance Offer and Coverage statement. Here you can view the form and print an additional copy.
